1. Complete a Youth Application form (pdf) or register on-line.

2. Complete the Cub Scout medical form. Include a copy of your scout's proof of medical insurance.

3. Any Adult planning to participate should complete an Adult Application form (pdf) or register on-line and complete the medical form.

​

4. Sign-up for Pack 445 communications.

​

5. Give medical forms and any other paperwork to pack leadership.

​
